PRACTICE DETAILS
In order for us to proceed, we would require all your Practice details, which would include your Practice Number, VAT number, a consent form for us to access your Remittance Advice's (This can either be accessed or emailed to clearmed2017@gmail.com)
and all your contact details.

PATIENT DETAILS/ACCOUNT RATES
Each account you wish to be submitted must be emailed to the email address provided (clearmed2017@gmail.com) along with the ICD10 codes (or diagnoses) and the procedure codes, any information on the admission form that is incorrect will be corrected by ourselves, if you wish to charge the account at a certain % you will need to notify us or the account will be processed at the specific scheme's rates.
